How can I write elements into HTML from MySQL database?

I need some directions on how to do it, 'cause I'm a bit lost.
I am inserting data into MySQL through a form, what I want to do is to automatically write a new item in my portfolio, how can I do this?My portfolio looks like this

  • what platform are you using? is this bespoke application or some kind of CMS, like Wordpress? are you using PHP?
